Patient Centered Medical Home

Patient-Centered Medical Home (PCMH) is a distinction earned by medical practices that emphasize coordination of care and communication between providers, patients, and their families. This patient-centered, whole-person approach to wellness is reflected in the way the office is organized, how the staff works together, and how they respond to the needs of our patients and families.

The National Committee for Quality Assurance (NCQA) provides third-party review and PCMH recognition according to evidence-based, nationally recognized clinical standards of care. Look for the NCQA PCMH seal on a physician or office.

What does PCMH mean for you?

When you seek treatment at a designated PCMH location, you can expect treatment in a calm, friendly, and personalized environment from providers who treat you with respect, compassion, and sensitivity. Our goal is to develop long-lasting, trusting relationships with better access to health care services to our patients and their families.

PCMH is a commitment to a high standard of quality that is reflected in our assurance to patients of reliability, accessibility, and professionalism. We strive to be conscious of the financial impact of the care on your family and the impact on our community and the environment.

A PCMH location is dedicated to treating you and your family in a comprehensive way with continuity and coordination of care among health care providers and settings. We help you focus on prevention and wellness through education and counseling as well as psychosocial and spiritual support.

Who is a PCMH patient?

As a patient, you play an active role in planning your care by communicating with your primary provider about your needs, concerns, medications, and other providers you might see.  We work with you to create a care plan that meets your specific needs and lifestyle. You take an active role in managing your wellness, seeking our help when you need it.
