We offer our patients, and their legal representatives convenient online access to obtain patient medical records without the need for printing, signing, and rescanning a release form. To streamline the process for our patients and their legal representatives, Arnot Health has created ONE CENTRAL release of information process for Arnot Ogden Medical Center, the St. Joseph's Hospital Campus, and Ira Davenport Memorial Hospital. You can now request your Arnot Health Provider's Medical Records as well as any one of the three Arnot Health Hospitals via the HIM team and MRO eXrpess.

All requesters must provide proof of identification or documented proof of legal relationship as appropriate (personal representatives, patient advocate, executor, trustee or guardian, requesting medical records on behalf of a patient will need to show proof, as well as identification, along with the medical record request form.
We believe your health information is personal. We keep records of the care and services that you receive at our facilities. We are committed to keeping your health information private, and we are required by law to respect your confidentiality.
We are dedicated to providing you your records timely. Medical record requests are processed in the order they are received and will be delivered to you as quickly as possible. For urgent needs, please contact the medical records department directly.
Depending on the purpose of your request, you may incur a fee for a copy of the records. You will be advised of any potential fees when your request is submitted and again before it is completed.
