Arnot Health has launched a new partnership to offer chronic disease management and specialty pharmacy services for patients living with chronic and complex conditions across the region. These new services, which are anticipated to be available to patients beginning this summer, will further Arnot Health's mission to deliver premier, patient-centered, and high-quality care in the Southern Tier of New York. Trellis Rx will embed their specialty pharmacists and patient liaisons at Arnot Health, to work with Arnot Health's care teams to coordinate with providers so that patients receive the high-touch support needed to manage complex medication regimens.
