Disease Prevention and Management
Chronic Disease Self-Management Program (CDSMP)
CDSMP is a 6-week workshop designed to empower individuals with chronic health conditions such as pain, asthma, diabetes, arthritis, high blood pressure, and COPD to feel better and to take control of their overall health. Workshop sessions are two and a half hours, once a week, for 6 weeks. Regular attendance is required to ensure the best results.
