What We Do
Transitional medical care and therapy can make a tremendous difference in overall recovery. During discharge planning, your doctor may refer you to the Transitional Care Unit for medical, skilled nursing, or rehabilitation services. Stays typically last 5 to 21 days.

How We Help
Located on the St. Joseph's Hospital Campus in Elmira, the Transitional Care Unit is a comfortable, healing environment. We offer private and semi-private rooms with specialized beds, Internet access, in-room meal delivery, community lounges, an inter-faith chapel and other amenities to encourage mobility and independence.

Admissions and Visiting Hours
